Video Reserve Request Form

Instructors may place a "hold" on library videos or DVDs to be used in the classroom later in the term. Please make requests at least 2 weeks prior to the date(s) needed, to ensure your item will be available. Videos/DVDs are checked out to instructors for three days, or as needed, and will be available at Circulation on the requested date. Please ask for your video/DVD by its call number when you come to pick it up.
